Resident Evil is known for its puzzles, some being easy to solve and some puzzles requiring some more brainwork.

Spoilers ahead as we’ve got the solution to the Organ Transport Box puzzle!
The Organ Transport Box puzzles is one of the trickier puzzles in the game, requiring several steps before the puzzle itself can even begin.
The first step is to acquire the Organ Transport Box itself, which is located in the garage but to access that you first need to find the wrench (located in the Custodian Office LVL 1).
When in the Garage head all the way to the back where you can find the Organ Transport Box chilling on the back of the cooling van. A small event will happen after (hint: shoot!)
After your epic standoff it’s time to open your Inventory and examine the Organ Transport Box and flip it over to its back, pressing the interact button to unlock a hidden puzzle.
Follow these steps to unlock the Puzzle:
- 1 – Flip the switch from AUTO to HAND
- 2 – Drag the dial halfway up
- 3 – Flick the dial on #5 to the halfway point
- 4 – Flick the dial on #2 to the halfway point
- 5 – Flick the switch from HAND to AUTO
If you somehow mess this up, there is also a RESET button on the far upper left that you can use to try again!
Now you unlocked the Organ Transport Box it’s time to get the LVL 2 ID Wristband.

Now with the Organ Transport Box fully unlocked it’s time to get the Artificial Lungs and Heart for the body that is located in the Blood Lab.
You acquire the Artificial Lungs just by opening the Organ Transport box, so now it’s time to get the Artificial Heart!
You can find the Artificial Heart in the Cold Storage, which is unlocked by using the Wrench on the locked door located near the Chairman’s Office, at the bottom of the stairs.
Grab the Heart in there along with some additional loot, and make your way back to the Blood Lab where you now can place the Artificial Lungs and Artificial Heart in the opened up body, resulting in a spooky 1V1.
When you eventually defeat the awakened patient you can collect the LVL 2 ID Wristband from it, giving you access to the only LVL 2 door; the Isolation Ward.
